Connect with a Family Doctor Through the Hamilton Family Health Team

Are you or someone you know in Hamilton, Haldimand, or Niagara Northwest looking for a family doctor? The Hamilton Family Health Team (HFHT) is here to help through their Patient Attachment Program.
 

Visit HFHT at the Hamilton Public Library

Every Wednesday from 10:00 AM to 2:00 PM, our friendly team will be at the Hamilton Public Library – Central Branch (55 York Blvd). Each week features a different healthcare professional available to answer your questions about health and how to connect with a family doctor.

If you don’t currently have a family doctor, you can connect with our Patient Attachment Program:

For Health and Community Partners

If you’re a hospital or community partner assisting patients in finding a family doctor, you can submit a referral through our secure form. Please note:
  • Patient consent is required
  • The referral form should be completed by health and community partners only
  • Do not share the referral link directly with patients
